BSA Health History Form (Part A&B)

Every child and adult attending camp must complete this form.  Turn in to your pack coordinator or the Day Camp Registrar.
Cub Scout Supplemental Medical Form (C3)

Every Scout and Tag-A-Long must complete this form.  Turn in to your pack coordinator or the Day Camp Registrar.
Youth Volunteer Supplemental Medical & Agreement

Every Youth Volunteer must complete this form.  Turn in to your pack coordinator or the Day Camp Registrar.
Completed enrollment and medical forms should be turned in to your pack's coordinator.  If you don't have one, mail completed forms to: Day Camp Registrar.
Youth Volunteer Recommendation Letter

First time Youth Volunteer must have this form completed by a scout leader or teacher.
​​​​​If you are working at a station that is being led by an outside presenter or a day camp staff member, there may not be a guide.  In those instances, the station leader/presenter will give you the information you need at the station.  

These guides will be at the station when you get there...so no need to print, just look them over.  Thanks!
